As more is understood about the long-term screening, surveillance, and symptom management needs of cancer survivors, ASCO and others are developing clinical guidelines to assist oncologists and other providers to address the ongoing care of this growing population.

Survivorship Care Plans
The 2006 IOM report, From Cancer Patient to Cancer Survivor: Lost in Transition, recommends that patients completing their first course of treatment be provided with a comprehensive care summary and follow-up care plan. Together, these two documents make up the Survivorship Care Plan (SCP). The SCP is a record of cancer and treatment history, as well as any checkups or follow-up tests needed. It may also list possible long-term effects of your treatments, and ideas for staying healthy. Additionally, the SCP can be used to enhance communication and coordination between the oncology team, patient, and PCP however, this document should be coupled with comprehensive, patient-specific screening and counseling. Below are templates and resources developed by ASCO and others designed to support effective communication of a survivor’s health status and long-term care needs.
